- Does it unlock another Operation or mission?

Those checks are more useful than assuming every marker can be cleared. The official structure allows opportunities to branch and disappear, so a completion route needs evidence from the exact campaign state and version.
What remains unverified
EA has not published a full table comparing every difficulty setting, a fixed count of cycles, or every consequence of disabling permadeath. This page therefore does not recommend a universal “best difficulty” or claim that one setting is required for a trophy.
